Why this matters

If one person is thinking about last week's crisis and another about the whole year, their scores can't be compared. Give everyone the same picture and the answers line up, so the patterns in your report are real, not noise.

Say which team. People often belong to more than one.
Say which work. Day to day, or one particular project.
Say what period. The last three months is a good default.
Leave out the problems you suspect. Naming them steers people's answers before they start. Save those for the next step.
